Inclus has now added incident reporting as a new feature. At the moment only Inclus staff can activate incidents to your project. Contact your inclus contact person to activate this feature.

Process for how to setup incident reporting form and start collecting incidents for your project:

Step 1 - creating the incident form

Go to the incidents tab in your project.

Start editing the form by adding new fields for filling information about the incidents. The default fields are form title and description.

Types of fields:

You can add basic text fields, Date and time fields, Multiple choice fields and File attachments.

You can edit the field name after you have added it to the form. It is also possible to make the field mandatory to answer with the slider.

Step 2, sharing the link:

When you have finished editing the incident reporting form, you can then share it to your organization. If you set the form as private, all reporters need Inclus account for filling the form. If the form is set open, you can share the link freely in different channels, for example in your companys intra page to your organisation and stakeholders.

Step 3, handling the reports:

You can monitor all of the incidents in a list mode and can see the amount of incidents as well their status.

By clicking the incident report label you can watch the detailed information about the incident:

Set the incident status to being handled when that incident handling has started.

Edit the status also in the main view by clicking the wrench icon.

Step 4: Connect the incident report for certain risk or items as well.

When editing the incident report internal information, you can connect the incident with certain risks or items in the risk management process.
