How do I set up an assessment questionnaire?

Questions in the questionnaires consist of assessment criteria for assessing the assessment targets (risk events, targets, change drivers, etc.). In principle, all criterias are consistently applied to all assessment points, so the criteria and assessment criteria should be given attention to compatibility.

 


Add existing assessment targets list to the risk/item tab. You can add risks copying them from Excel as a batch. 





Activating risks from identification to assessment: 


If you have used risk identification session, and there is new suggestions for risks, go through those and activate the most relevant for your process to the risk list. 




Edit the risk name and description if necessary before adding it to the risk list. Risk can be tagged at the same time as well, for example when filtering certain risks to the assessment form. 





You can define assessment criteria to each session separately.  On the project page tabs, click Criteria

Setting up background groups 



Edit session content if necessary. 

You can filter content from assessment questionnaire from session page clicking the edit session icon. 

Filter what items/risks, answer groups or criteria you want to choose or leave out from the questionnaire. If there is no filters chosen, all the content will be shown in the questionnaire. 


 

In addition, you can add open questions to different sections of your questionnaire. On the project page, click Open Questions tab.
